RIVIAM’s dynamic forms allow referral information to be captured quickly and easily online

When there is a professional or citizen on the phone, RIVIAM’s dynamic forms allow information to be captured about a request quickly.

The forms can be completed by a user within RIVIAM and once the information is submitted, it’s then available for triage.

The forms can be changed to meet service requirements.
