RIVIAM integrates with the hospital's EPR to update the Discharge Dashboard with the latest information about a person's ward and Estimated Discharge Date.

This data is then securely shared with community and third sector providers using RIVIAM's Multi-agency Referral Hub giving them greater confidence about when to visit a person.
